The Eye Center is committed to providing you the highest quality comprehensive eye care services, in order to provide this level of service we have made retinal imaging a necessary requirement that we have set at an affordable $39. The retinal image takes a quick picture of the inside of the eye which allows the doctor to check the overall health of the eye with no side effects!

We require confirmation of your appointment no later than 3:00PM of the day prior to your appointment, failure to do so may result in your appointment being cancelled due to non-communication. You will receive an appointment confirmation via phone or email. Please confirm your appointment as instructed to ensure your slot is secured.

Please be on time or early for your appointment. The Eye Center has a strict 15 minute grace period for your appointment time, if you do not arrive before the 15 minutes we would have to reschedule your appointment.

Prior to your exam, please fill out the patient intake form below prior to your exam date to avoid excessive wait times.

What Do I Bring With Me?

Valid Photo ID: A driver’s license, state ID, or passport to verify your identity.
Insurance Information: Your current health insurance card(s) and any relevant insurance documents. This allows us to verify coverage and process any claims efficiently.
Most Recent Eyeglass Prescription: Even if you primarily wear contact lenses. This helps us assess your current prescription and how we can improve your visual needs.
Contact Lenses: If you are a contact lens wearer, bring your current contact lenses and the packaging or prescription details. It's helpful to know the brand, prescription, and how often you wear them.
List of Medications: A current list of all medications and supplements you are taking, including dosages. This information is crucial as certain medications can affect your vision and eye health.
Medical History: Any relevant medical records, especially if you are a new patient or have had recent eye surgeries or treatments. This includes previous eye exam records if available.
Payment Method: For co-pays, deductibles, or services not covered by insurance. We accept various payment methods, including credit cards, checks, and cash.
